Your
clients are frequently asked to provide proof of insurance
to customers, vendors and municipalities. To request
a certificate, simply fax your request to Northwest Customer Service at the number below
with the following:
Name
and address of the Insured, name of the certificate
holder, their mailing address, the contact person,
their interest (i.e. Client, Franchisor, Prop. Mgr.,
Gen. Contr.), along with their phone & fax numbers.
Include a copy of their “insurance requirements”
if available.
