Supplemental application relevant to the type of business.
Statement of qualifications and resumes of all key personnel.
Latest audited financial statement.
Professional Liability Application for specified Professions.
FIDICIARY LIABILITY:
Please complete the private company protection plus form.
Please provide your Fiduciary Plan form 5500
Complete Sections 1, 4, and 5 of application along with pertinent client information.

TO BIND:
Upon receipt of faxed documents and a request to bind, NWC will review for accuracy and bind coverage on accounts where we have binding authority and request that the policy be bound on those quotes we don't have binding authority.
NWC will notify the agent that coverage has been bound.
When policy is received from carrier, NWC will review policy for accuracy.
NWC will mail a copy of policy to the agent.
NWC will process all endorsement requests and other changes or inquiries that occur during the policy period.
